RN - Case Management

Job Title: RN -Case Manager 
Pay range - $65/hr - $70/hr.
Work Hours: 40 hours/week (8-hour shifts)
Shift Timing: 9:00 AM – 5:00 PM
Workdays: Monday – Friday
Location - Bronx, NY 
 
Job Summary:
The Case Manager – RN will be responsible for providing comprehensive case and resource management, ensuring proper utilization of services and continuity of care. The ideal candidate will coordinate patient needs through assessments and collaboration with interdisciplinary teams using evidence-based care planning and documentation.

Responsibilities:

• Perform case/resource management and utilization review
• Assess patient needs and coordinate appropriate care plans
• Collaborate with healthcare providers and support staff to ensure effective service delivery
• Document patient interactions and outcomes in EPIC
• Use tools like PRI (Patient Review Instrument) and SCREEN for patient evaluations

Skills & Requirements:

• Experience: Minimum 2 years in case/resource and utilization management (Required)
• Care Management Experience: 3–5 years (Preferred)
• EPIC Experience: Required
• PRI & SCREEN Certification/Experience: Ideal
• Education: Registered Nurse (RN) License required
• Start Date: April 28, 2025
