Job Title: Care Manager | UAS Assessor RN
Location: Bronx, Manhattan, or Westchester, NY (Remote + Field Visits)
Duration: 3 Months+ (Potential for Contract-to-Hire)
Pay Range: $50 – $54.13/hr. on W2
Shift Time: Monday to Friday, 8:30 AM – 5:00 PM EST
Position Overview:
- True/Correct Title: UAS RN
- Employment Type: Temp-to-Hire
- Work Model: Hybrid — Field visits in Bronx, Westchester, or Manhattan.
- Travel: RNs will visit members in their homes, assisted living facilities, and nursing homes.
- Travel Reimbursement: Mileage, tolls, parking fees, and ride-sharing services covered.
- Caseload: Minimum of 15 cases per week.
- Orientation: 5 days for current UAS-certified RNs; longer for those new to UAS.
- Language Requirements: Chinese, Spanish, or Bengali.
Key Responsibilities:
- Conduct face-to-face assessments of members’ functional status, medical, behavioral, psychosocial, and community resource needs.
- Provide the Interdisciplinary Care Team (ICT) with assessment findings and act as a facilitator to ensure members’ needs are met.
- Develop an Individualized Care Plan (ICP) / Patient-Centered Service Plan (PCSP) based on clinical, behavioral, and social needs, addressing barriers to care.
- Assess members’ health status to ensure all necessary medical and supportive services are in place.
- Modify ICP/PCSP as members’ needs and progress change.
- Manage care transitions with effective, timely communication to support member care and discharge planning.
- Clarify plan medical benefits, policies, and procedures for members, providers, and community-based agencies.
- Prepare for and participate in clinical case reviews to share best practices.
- Adhere to documentation policies and procedures, ensuring clinical assessment activities and their effectiveness are properly recorded.
- Maintain comprehensive working knowledge of community resources, payer requirements, and network services for the target population.
- Collaborate with other Care Managers to reduce practice variations, identify system improvement opportunities, and help develop relevant policies and procedures.
- Provide backup to other assessment nurses as needed.
- Analyze data to improve care delivery.
- Perform other duties as assigned.
Required Experience:
Management will accept any of the following:
- UAS-certified RN licensed in New York State.
- New graduate RNs (will be considered).
- Non-UAS RNs with experience in home care management.
Required Education:
- Associate’s or Bachelor’s Degree in Nursing (a combination of relevant experience and education will be considered in lieu of degree).
Required Licensure/Certification:
- Active New York State Registered Nurse (RN) License.
Skills & Competencies:
- Strong professional knowledge and comprehensive clinical assessment skills for the adult population and chronic disease management.
- Excellent verbal and written communication skills.
- Proficient in Microsoft Office (Outlook, Word, Excel).
- Ability to work independently and adapt to a fast-paced environment.
- Skilled in data analysis to improve care delivery.
- Self-starter with high accountability and responsibility for care outcomes.
- Highly organized; able to manage multiple priorities effectively.
- Strong problem-solving and independent decision-making skills.
- Collaborative, relationship-building mindset with providers, members, and the multidisciplinary team.
