Weekend Requirement: Yes, Every Other Weekend
Weekly Guaranteed Hours: 48 hours preferred; 36 hours acceptable
Call Requirement: None
Float Requirement: May float based on skillset
Unit Overview:
- Unit Type: Transitional Care / Post-Acute Unit
- Number of Beds: 46 (at capacity)
- Patient Ratios: Maximum 1 RN : 8 Patients

Patient Population:
- Mix of medical and surgical patients
- Medically ready for discharge
Key Responsibilities:
- Deliver high-quality, patient-centered nursing care in a transitional/step-down setting
- Manage patient assessments, care planning, interventions, and evaluations
- Document care and patient responses accurately in Epic EMR
- Collaborate with interdisciplinary teams to coordinate discharge planning
- Maintain a strong focus on patient safety, dignity, and rehabilitation
- Communicate effectively with patients, families, and team members
- Use mobility and monitoring equipment proficiently (see below)
Required Qualifications:
- Active RN License (State-specific – insert state if known)
- Basic Life Support (BLS) – Required
- Minimum of 1 year recent RN experience in acute care or transitional care settings
- EMR experience required – Epic strongly preferred
Preferred Qualifications:
- Recent transitional care, skilled nursing, or rehab unit experience
- Familiarity with complex discharge and post-acute planning
Unit Equipment Includes:
- Vital Monitoring: Philips Monitor Vital Sign Machines

Mobility/Patient Support:
- Overhead lifts, Sara Steady, Hover Mats
- Standing Scales, Slide Boards, Bladder Scanner
- Sage Mats, Synergy Air Beds, Stryker Beds
- Stretchers, Wheelchairs, Walkers
