- Job Title: RN – Case Manager
- Location: Novato, CA
- Duration: 3 +Months (Possibility of extension)
- Shift Time: Day Shift | 08:30 AM – 05:00 PM | 5 shifts/week | Every other weekend & holidays required
- Local Pay Range: $70/hr on W2 (Negotiable)
- Travel Pay Range: $3000.00/Weekly blended with Stipend (Negotiable)
Position Summary
We are seeking a dedicated RN Case Manager with strong clinical judgment and care coordination experience. The RN will work in a skilled nursing/post-acute setting, ensuring high-quality patient outcomes while collaborating across interdisciplinary teams. This role requires flexibility to float between multiple facilities (7 locations within a 30-mile radius).
Key Responsibilities
- Provide comprehensive case management and discharge planning for patients in skilled nursing and post-acute settings.
- Collaborate with physicians, therapists, social workers, and families to develop individualized care plans.
- Perform clinical assessments, document progress, and ensure compliance with regulatory and facility standards.
- Support transitions of care, ensuring continuity and quality across facilities.
- Float between assigned facilities as needed, maintaining adaptability and consistent standards of care.
Required Qualifications
- Active California RN license (unrestricted).
- 1+ year recent case management experience in a skilled nursing facility or post-acute care environment.
- Strong knowledge of discharge planning, utilization review, and interdisciplinary care coordination.
- Excellent communication, critical thinking, and organizational skills.
- Flexibility to float across multiple facilities (within 30 miles).
