Seeking a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join the Case Management team. This role promotes autonomy and flexibility while coordinating care for members. The coordinator is responsible for telephonic and face-to-face assessment, planning, implementation, and coordination of case management activities to evaluate medical needs and facilitate overall wellness.

- Monday – Friday, 8:00 AM – 5:00 PM (Standard Business Hours)
- Candidate must adhere to schedule
- Travel approximately 85% of the time within Orange, Osceola, Brevard, or Seminole counties to visit members at homes, Assisted Living Facilities, and Skilled Nursing Facilities
Position Summary:
Seeking a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join the Case Management team. This role promotes autonomy and flexibility while coordinating care for members. The coordinator is responsible for telephonic and face-to-face assessment, planning, implementation, and coordination of case management activities to evaluate medical needs and facilitate overall wellness.
Requirements:
- Must reside in Central Florida (Orange, Osceola, Brevard, or Seminole Counties)
- Bilingual (Spanish/English) – fluent in speaking and writing
- Ability to multitask, prioritize, and adapt effectively in a fast-paced environment
- Effective communication skills, verbal and written
- RN with current unrestricted Florida licensure or valid COMPACT license
- Case Management Certification (CCM) preferred
- Candidates must not be nurses for non-nursing roles (unless RN licensure required)
- Safety-sensitive (field and telephonic)
- Eligible to submit mileage
Duties:
- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees
- Conduct comprehensive evaluation of members using care management tools and data review
- Collaborate and facilitate case management process to ensure appropriate healthcare outcomes
- Implement assigned care plan activities and monitor progress
- Conduct multidisciplinary reviews to achieve optimal outcomes
- Identify and escalate quality-of-care issues through established channels
- Utilize negotiation, motivational interviewing, and coaching skills to engage members
- Educate members on healthcare options, medications, and community resources
- Monitor, evaluate, and document care in compliance with regulatory and accreditation guidelines and company policies
- Promote member engagement in healthcare decision-making and healthy lifestyle choices
Experience:
- Minimum 3 years clinical practice experience (hospital, home health, ambulatory care)
- Healthcare and/or managed care industry experience
- Case management experience preferred
- Proficiency with computer systems and keyboarding
Education:
- RN with current unrestricted Florida license or valid COMPACT license
- Case Management Certification (CCM) preferred
