Integrated Case Manager

Job Summary

The Integrated Case Manager for Population Health is a key member of the patient-centered care or treatment team. This role involves collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy to address comprehensive healthcare needs of individuals and families. The goal is to promote patient safety, quality care, and cost-effective outcomes by utilizing available resources and effective communication.

This position focuses on patients who have experienced critical events or complex diagnoses requiring extensive management strategies and resource use across the care continuum. Services are provided across ambulatory, inpatient, and health plan settings.




Principal Duties and Responsibilities

  • Conduct comprehensive assessments of patients and families biomedical, psychological, social, and functional needs to understand their impact on recovery.
  • Develop personalized, patient-centered care plans to optimize the care experience.
  • Engage patients and families as active members of the care team through advocacy, communication, health education, and resource facilitation.
  • Apply professional judgment, critical thinking, motivational interviewing, and self-management techniques to help patients overcome barriers to achieving goals.
  • Provide counseling on treatment decisions and end-of-life issues, including Advanced Care Planning.
  • Coordinate patient transitions between care settings to ensure seamless and safe care.
  • Advocate for appropriate service delivery within the patients health plan benefits.
  • Collaborate with treatment/care team members to co-manage patients with complex medical and social needs, facilitating collaborative care conferences.
  • Continuously evaluate patient responses to care plans and adjust as needed.
  • Participate in process improvement activities to reduce risk, including data collection, analysis, and follow-up.
  • Facilitate interventions in cases involving child abuse, neglect, domestic violence, elder abuse, institutional abuse, and sexual assault.
  • Support departmental goals contributing to organizational success.
  • Perform other duties as assigned.


Required:

  • Nursing degree OR
  • Master of Social Work (MSW)


Required:

  • Registered Nurse (RN) with a valid, unrestricted Michigan State license OR
  • Licensed Master Social Worker (LMSW) with a valid, unrestricted Michigan State license



Skills & Experience

Required:

  • Excellent verbal communication and written documentation skills
  • Strong customer service and interpersonal skills, able to interact effectively at all organizational levels
  • Strong problem-solving, analytical, and decision-making skills
  • Proficient computer skills and knowledge
  • Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care preferred
  • Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare and Medicaid regulations, and case management principles
  • Understanding of medical ethics and legal issues related to case management
  • Awareness of social determinants of health and their impact on patient wellbeing
  • Skilled in facilitating community resources to support diverse populations
  • Strong organizational, planning, and multitasking abilities to manage complex patient needs
  • Compassionate with a strong ability to advocate effectively for patients and families

  • Location: Detroit, MI
  • Start Date: April 28, 2026
  • End Date: July 25, 2026
  • Duration: 13-week contract
  • Schedule: 40 hours per week, 8 hours per day, 5 days per week with weekend rotation



Additional Notes

  • Must have 5+ years of case management experience with at least 2 recent years in acute inpatient case management.
  • BSN or MSW required.
  • Current Michigan RN or LMSW licensure required.
  • Discharge planning experience essential.
  • Limited onsite training; candidates must be able to adapt quickly.
  • Manager will contact candidates directly for interviews. Candidates in Client Review status should be prepared.
  • Manager enforces strict requirement for recent inpatient case management experience. Telephonic, utilization review, or psychological experience does not meet this requirement.
