Job Title: Registered Nurse (RN) Case Management
Location: Palmdale, CA
Duration: 3 Months+
Shift: 8:30 AM – 5:00 PM or 9:00 AM – 5:30 PM, One weekend shift per month required
The Payrate Information:
Overall Weekly Pay Rate: $2800
W2 Weekly Pay Rate: $1,540
Weekly Stipend: $1,260
Requirements:

Requirements:
- CA State RN license
- 2+ years of experience of Case Management in Acute Settings.
- BLS from AHA.
- The Case Management RN is responsible for coordinating and facilitating comprehensive patient care through assessment, planning, implementation, and evaluation.
- Conduct assessments to identify patient needs and develop individualized care plans
- Coordinate transitions of care and discharge planning
- Collaborate with physicians, social workers, and other healthcare professionals to ensure effective care delivery
- Monitor patient progress and adjust care plans as needed
- Advocate for patient needs and facilitate access to appropriate services and resources
- Ensure compliance with regulatory requirements and organizational standards
- Document all case management activities accurately and timely
