Job Title: Registered Nurse (RN) - Case Manager
Locations: Santa Rosa, CA 95403
Duration: 3 Months+ (Possible Extension)
Shift: 7:00 AM to 03:30 PM
Pay Range: $60 - $70/hr. on W2
Stipend available for travelers with distance >50 Miles

Required Qualifications:
- Active California RN license (unrestricted).
- 1+ year current case management experience in a Acute Care/hospital setting.
- BLS & ACLS (AHA)
Responsibilities:
- Provide comprehensive case management and discharge planning for patients in skilled nursing and post-acute settings.
- Collaborate with physicians, therapists, social workers, and families to develop individualized care plans.
- Perform clinical assessments, document progress, and ensure compliance with regulatory and facility standards.
