Shift: Monday Friday, 8 AM 5 PM EST
Location: Arlington, TX, 76001
Travel: Up to 75% local travel within Denton, Flower Mound, Keller, Arlington, Mansfield, Fort Worth, Dallas, Ellis, Navarro, Kaufman, Rockwall, Hunt, Collin, and/or Tarrant counties
Position Summary
The Nurse Case Manager is responsible for telephonic and face-to-face assessment, planning, implementation, and coordination of case management activities. The goal is to evaluate members medical needs, promote overall wellness, and facilitate safe, cost-effective care.
The role requires a collaborative approach that integrates assessment, planning, facilitation, advocacy, and coordination of care while ensuring compliance with regulatory and accreditation standards.

Key Responsibilities
- Conduct comprehensive member assessments (telephonic and in-person) to determine medical needs and care requirements
- Develop and implement individualized care plans, monitoring and adjusting as needed
- Active, unrestricted RN license in the state of Texas
- Minimum 3+ years of clinical experience
- At least 1+ year of case management experience
