Position Summary
- The Nurse Case Manager uses a collaborative process of assessment, planning, facilitation, and advocacy to meet members health needs and promote optimal, cost-effective outcomes.
- This role involves both telephonic and face-to-face interactions and requires strong clinical judgment, case management expertise, and excellent communication skills.
- Assess, plan, implement, and coordinate case management activities for members to improve overall wellness.
- Develop proactive care plans addressing short- and long-term member needs.
- Apply network management and clinical coverage policies to guide service delivery.
- Monitor care plan progress and adjust as needed.
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of Texas.
- 3+ years of clinical practice experience.
- 1+ year of case management experience.
- Comfortable with extensive local travel (up to 1 hour per trip).
