Job Title: RN Case Manager
Job Location: Fairfield, OH 45014
Job Duration: 3+ Months (Possibilities of extension)
Shift: Monday - Friday, 8:00am - 5:00pm EST
Payrate: $35.95/ hr on w2

Butler, Clermont, Clinton, Hamilton, Warren Counties for in person visits with 75% travel.

Job Summary:

• The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate services that meet an individual’s benefit plan and/or health needs.
• Through communication and available resources, the Case Manager promotes optimal, cost-effective outcomes.

Experience & Skills:

• Minimum 3 years of clinical practice experience (hospital, home health, ambulatory care, or other alternative care setting).
• Experience in healthcare and/or managed care industry.
• Case management experience preferred.

Education & Credentials:

• RN with current, unrestricted state licensure.
• Case Management Certification (CCM) preferred.
