Job Title: Registered Nurse (RN) - Case Manager
Locations: Oakland, CA 94611
Duration: 3 Months+ (Possible Extension)

Shift: 09.00 PM – 05.00 AM
Rate: $70 Local | $85 Travel 


Key Responsibilities:

• Provide comprehensive case management and discharge planning for patients in skilled nursing and post-acute settings.
• Collaborate with physicians, therapists, social workers, and families to develop individualized care plans.
• Perform clinical assessments, document progress, and ensure compliance with regulatory and facility standards.
• Support transitions of care, ensuring continuity and quality across facilities.
• Float between assigned facilities as needed, maintaining adaptability and consistent standards of care.
   

• Active California RN license (unrestricted).
• 1+ year recent case management experience in a skilled nursing facility or post-acute care environment.
• BLS & ACLS (AHA)
