Job Title: Case Manager (Remote + Local Travel)
Pay Rate: $41.29/hr (W2)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Locations Hiring: Cincinnati area.
(Remote role with required local home visits to members)

Summary
Responsible for healthcare management and coordination of members to achieve optimal clinical, financial, and quality-of-life outcomes. Works collaboratively with members, families, and healthcare providers to develop, implement, and monitor individualized care plans.
Essential Functions
Provide case management services to members with chronic or complex conditions.
Conduct comprehensive assessments using data from internal systems, members, caregivers, and providers.
Develop and implement individualized plans of care in collaboration with the interdisciplinary team.
Conduct home visits and follow up to evaluate effectiveness and adjust care plans.
Document all activities accurately in client systems and maintain compliance with policies, procedures, and regulations.
Promote integration of services (behavioral health, long-term care, etc.) for continuity of care.
Meet or exceed productivity, quality, and compliance standards.
Participate in staff meetings, interdisciplinary care team discussions, and mentoring of new team members.
Maintain professionalism, confidentiality, and adherence to HIPAA and Code of Conduct.
Knowledge, Skills, and Abilities
Strong communication, problem-solving, and organizational skills.
Ability to manage multiple priorities and work independently.
Knowledge of ICD-9/CPT coding, SSI, COB, TPL programs, and case management tools (e.g., CCA).
Familiarity with NCQA standards, and state/federal healthcare regulations.
Proficient in Microsoft Office and case management documentation systems.
Understanding of HIPAA and patient confidentiality standards
Required Education:
Bachelor’s degree in Nursing OR
Master’s degree in Social Work or Health Education
(Equivalent combination of education and experience may be considered)
Experience:
0–2 years of clinical experience, with prior case management preferred.
Licensure / Certification
Active, unrestricted RN, LSW, or LISW license in the state of Ohio (in good standing).
Valid driver’s license with good driving record and ability to travel locally.
