Job Title: Integrated Case Manager
Location: Detroit, MI 48202
Duration: 5 Months+ (Possible Extension)
Shift: 8:00 AM – 4:30 PM (Weekend rotation required)
Schedule: 40 hours/week (8 hours/day, 5 days/week)
Job Summary:
The Integrated Case Manager for Population Health is a key member of the patient-centered care team. This role is responsible for assessment, planning, care coordination, evaluation, and advocacy to meet the comprehensive healthcare needs of patients and their families. The position focuses on supporting patients with complex conditions requiring extensive resource utilization across the care continuum, including ambulatory, inpatient, and health plan settings.
Key Responsibilities:
- Conduct comprehensive assessments of patients and caregivers, including biomedical, psychological, social, and functional needs
- Develop individualized, patient-centered care plans
- Engage patients and families through education, communication, and advocacy
- Utilize critical thinking, professional judgment, and motivational interviewing to address barriers to care
- Provide counseling related to treatment decisions and end-of-life planning, including advance care planning
- Coordinate safe and effective transitions between care settings
- Advocate for appropriate services within the patient’s health plan benefits
- Collaborate with multidisciplinary teams to manage complex cases and facilitate care conferences
- Monitor and evaluate patient progress, adjusting care plans as needed
- Participate in quality improvement and risk reduction initiatives
- Address and intervene in cases involving abuse, neglect, or violence
- Support departmental and organizational goals
- Perform other duties as assigned
Education Requirements:
Required:
- Nursing degree OR Master of Social Work (MSW)
Certifications & Licensure:
Required:
- Registered Nurse (RN) with an active, unrestricted Michigan license OR
- Licensed Master Social Worker (LMSW) with an active, unrestricted Michigan license

Skills & Experience
Required:
- Strong verbal and written communication skills
- Excellent interpersonal and customer service skills
- Strong analytical, problem-solving, and decision-making abilities
- Proficiency in computer systems and documentation
- Knowledge of:
- Discharge planning, home health, rehab, community health, or managed care
- Preventive service and clinical practice guidelines
- Behavior change theory
- Medicare and Medicaid regulations
- Case management principles
- Medical ethics and legal considerations
- Social determinants of health
- Ability to coordinate community resources for diverse populations
- Strong organizational and multitasking skills
- Compassion and patient advocacy mindset
Requirements:
- 5+ years of case management experience
- 2+ years of recent acute inpatient case management experience
- BSN or MSW
- Current Michigan RN or LMSW license
- Discharge planning experience
- Limited on-site training; candidates must adapt quickly
- Manager will contact candidates directly for interviews
- Candidates in “Client Review” status should be prepared for outreach
