Location: Atlanta, GA 30305
Duration: 3+ Months (Possible Extension)
Shift: 8 AM to 5 PM
Unit: Mental Health/Psych department
License/Certificate:
Required Experience:
Summary:
RN Case Manager supporting the Mental Health/Psych unit, providing coordinated patient care, assessments, and ongoing case management for behavioralhealth patients.
Responsibilities:
Location: Atlanta, GA 30305
Duration: 3+ Months (Possible Extension)
Shift: 8 AM to 5 PM
Unit: Mental Health/Psych department
License/Certificate:
- Active Georgia RN/compact license
- BLS – AHA
Required Experience:
- Current 1 year of RN case management experience in Psych/Mental Health setting
- Schedule: Rotating weekends
Summary:
RN Case Manager supporting the Mental Health/Psych unit, providing coordinated patient care, assessments, and ongoing case management for behavioralhealth patients.
Responsibilities:
- Conduct comprehensive mentalhealth assessments and develop individualized care plans for assigned patients.
- Coordinate treatment services, referrals, and followup across multidisciplinary behavioralhealth teams.
- Monitor patient progress, ensure continuity of care, and provide crisis intervention as needed, including rotating weekend coverage.
