Job Title: Registered Nurse (RN) - Case Manager
Location: Atlanta, GA – 30305
Contract: 3 Months+ (Possible Extension)
Shift: Mon - Fri | 8:00 AM – 5:00 PM (5x8hr shifts)
Pay Local: $60 - $70/hr-W2
Pay Travel: $2,500 - $3,000 / Gross weekly

Job Duties:
- Coordinate patient care plans in collaboration with physicians, social workers, and interdisciplinary teams.
- Conduct comprehensive assessments to identify patient needs, discharge planning requirements, and potential barriers to care.
- Monitor and evaluate the effectiveness of patient care plans and modify as needed.
- Facilitate transition of care by arranging follow-up appointments, home health services, DME, or rehab placements.
- Educate patients and families on treatment plans, discharge instructions, and community resources.
- Ensure compliance with regulatory guidelines, documentation standards, and hospital policies.
- Serve as a liaison between patients, providers, insurance companies, and community services.
- Manage utilization review to support appropriate level of care and reduce length of stay.
Requirements:
- Active Registered Nurse (RN) license in the state of Georgia (unencumbered).
- Minimum of 1 years recent experience in acute care case management.
- Strong knowledge of discharge planning, care coordination, and utilization review.
- Experience with EMR systems (e.g., Epic, Cerner) preferred.
