Job Title: RN Case Manager
Job Duration: 3+ Months (Possibilities of extension)
Shift: Monday-Friday 8a-5p EST
Location: Cincinnati, OH 45211
Cincinnati - Butler, Clermont, Warren, & Hamilton Co. areas, face to face member visits required.
Position Summary
- The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to coordinate services that meet an individual s benefit plan and/or health needs.
- Through communication and available resources, the Case Manager promotes optimal, cost-effective outcomes.
- Minimum 3 years of clinical practice experience (hospital, home health, ambulatory care, or other alternative care setting).
- Experience in healthcare and/or managed care industry.
- Case management experience preferred.
Education & Credentials
- RN with current, unrestricted state licensure.
- Case Management Certification (CCM) preferred.
