Transition of Care Registered Nurse
Job Title: Transitions of Care Nurse (RN or LPN)
Location: Fully Remote – Must reside in Northern Virginia (NOVA Region)
Work Hours: 8:00 AM – 5:00 PM EST
Duration: Contract with potential for extension
Time Zone Requirement: Eastern Standard Time (EST)
Role Overview:
The Transitions of Care Nurse plays a vital role in supporting members as they transition from inpatient acute care settings to either their homes or skilled nursing facilities (SNFs). This position ensures a smooth and safe discharge process, helping to improve member outcomes and reduce hospital readmissions.
Key Responsibilities:

Coordinate and support discharge planning for members transitioning from acute care settings.

Collaborate with medical teams, facilities, and member caregivers to ensure proper follow-up care.

Assess member needs post-discharge and help facilitate appropriate care plans.

Serve as a liaison between providers and members to ensure a seamless transition of care.
Qualifications & Requirements:

Licensure: Active RN or LPN license required.

Experience: Strong background in medical/physical health. Experience with Transitions of Care and Discharge Planning is a must.

Location: Must be located in the Northern Virginia (NOVA) region.
