Travel: RN Case Manager
Job Title: Travel RN Case Manager
Location: 200 Abraham Flexner Way, 2nd Floor, Louisville, KY
Duration: Duration: (Not specified)
Shift: Monday - Friday, 8:00 AM - 4:30 PM (No on-call or weekends)
Schedule: 5 days per week, 8 hours per day
Job Summary:
Coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness through the integration of functions of case management, utilization review and management, and discharge planning. Serves as a patient advocate through resource utilization, discharge planning, and addressing the holistic needs of the patient.
Qualifications & Skills:
Education:

Registered Nurse (RN) with an active Kentucky RN license (must be attached to resume).
Licensure & Certifications:

Must have BLS certification through the American Heart Association (RQI certifications will not be accepted).
Required Skills & Experience:

Minimum of 2 years of RN Case Management experience.

Experience in a long-term acute care setting, including discharge planning, clinical case management, and utilization review.
