LPN LTC
Shift - 3P-11P SHIFT NEEDED 11P-7A SHIFT NEEDED Able to work every other weekend MUST BE LOCAL
Pay range - $33/hr - $38/hr.
Job Summary:
-
Performs tasks and responsibilities within the framework of case finding, provision of supportive and restorative care under the direction of a Registered Professional Nurse.
-
Performs nursing skills and provides nursing care to an assigned group of residents in accordance with the policy of the facility.
-
Augments the quality of life of the residents.
-
The LPN will function as a member of the nursing team and the interdisciplinary team.
Job Duties:
Provides direct nursing care to assigned residents with complex needs as related to:
-
Hygiene and physical comfort
-
Medical treatment
-
Nutritional needs
-
Ambulation and exercise
-
Elimination
-
Maintenance of residents safety
Other duties:
-
Directs the care of the residents performed by Certified Nursing Assistants.
Administers medication to assigned residents with exception of the following:
-
Blood and blood products
-
Hanging the first IV solution containing medications
-
Administering IV (push) medication
-
Administering IV medications via central lines and maintenance
Administers IV therapy with exceptions mentioned above:
-
LPN s certified in IV therapy by Schulman and Schachne may perform:
-
Venipuncture for the purpose of placing a heparin lock or administering IV fluids
-
Set up and assist in starting IV fluids
-
Hanging fluids and monitoring fluid rate as presented by physician order
-
Replacing bags of IV fluids
-
Changing intravenous dressing, tubing on peripheral lines
-
Discontinue IV fluids
-
-
Administers treatment according to facility policy and procedure.
-
Prepares residents for diagnostic tests, operative procedures, medical regimen examinations, and transportation.
-
Admits residents according to facility and unit policies and procedures and assists them in adjusting to the environment.
-
Participates in the Person Centered Care planning and coordinating of the nursing care in an assigned unit and implements plan.
-
Assists and/or contributes to the development and updating of the Person Centered Care Plan to reflect the residents needs.
-
Assists RN with health teaching for residents and families as needed.
-
Establishes positive relationship with residents and families in accordance with Residents Bill of Rights.
-
Participates in unit/team ICP meetings and PI teams.
-
Observes nutritional needs of residents. Actively participates at mealtime by assigning staff, feeding residents, monitoring intake of residents consumption.

Assists in post-mortem care of deceased residents as needed.

Demonstrates competence and experience in nursing skill within the product of practice.

Has the ability to establish positive team morale and good interpersonal relationships with residents, families, and co-workers.

Licensed and currently registered as a Practical Nurse in New York State

BLS (American Heart Assoc.) or NY State CPR cert
