Shift - 3P-11P SHIFT NEEDED 11P-7A SHIFT NEEDED Able to work every other weekend MUST BE LOCAL
Pay range - $33/hr - $38/hr.

Job Summary:

• Performs tasks and responsibilities within the framework of case finding, provision of supportive and restorative care under the direction of a Registered Professional Nurse.

• Performs nursing skills and provides nursing care to an assigned group of residents in accordance with the policy of the facility.

• Augments the quality of life of the residents.

• The LPN will function as a member of the nursing team and the interdisciplinary team.

Job Duties:

Provides direct nursing care to assigned residents with complex needs as related to:

• Hygiene and physical comfort

• Medical treatment

• Nutritional needs

• Ambulation and exercise

• Elimination

• Maintenance of residents safety

Other duties:

• Directs the care of the residents performed by Certified Nursing Assistants.

Administers medication to assigned residents with exception of the following:

• Blood and blood products

• Hanging the first IV solution containing medications

• Administering IV (push) medication

• Administering IV medications via central lines and maintenance

Administers IV therapy with exceptions mentioned above:

• LPN s certified in IV therapy by Schulman and Schachne may perform:

  • Venipuncture for the purpose of placing a heparin lock or administering IV fluids

  • Set up and assist in starting IV fluids

  • Hanging fluids and monitoring fluid rate as presented by physician order

  • Replacing bags of IV fluids

  • Changing intravenous dressing, tubing on peripheral lines

  • Discontinue IV fluids

• Administers treatment according to facility policy and procedure.

• Prepares residents for diagnostic tests, operative procedures, medical regimen examinations, and transportation.

• Admits residents according to facility and unit policies and procedures and assists them in adjusting to the environment.

• Participates in the Person Centered Care planning and coordinating of the nursing care in an assigned unit and implements plan.

• Assists and/or contributes to the development and updating of the Person Centered Care Plan to reflect the residents needs.

• Assists RN with health teaching for residents and families as needed.

• Establishes positive relationship with residents and families in accordance with Residents Bill of Rights.

• Participates in unit/team ICP meetings and PI teams.

• Observes nutritional needs of residents. Actively participates at mealtime by assigning staff, feeding residents, monitoring intake of residents consumption.

• Assists in post-mortem care of deceased residents as needed.

Required Skills & Experience:

• Demonstrates competence and experience in nursing skill within the product of practice.

• Has the ability to establish positive team morale and good interpersonal relationships with residents, families, and co-workers.

Preferred Skills & Experience:

• 1+ years relevant experience

Required Education:

• Completion of LPN program

Required Certifications & Licensure:

• Licensed and currently registered as a Practical Nurse in New York State

• BLS (American Heart Assoc.) or NY State CPR cert
