Job Title: Medical Management Specialist I
Location: Remote + Field Visits near Farmington, NM
Shift: Full-time 40hrs, Day Shift 8am to 5 pm
Role Overview:
This position supports members on government health care programs through medical management and health education. The Case Manager will conduct remote and in-person visits, assess member needs, and coordinate care. The role also involves compiling data for regulatory reporting and serving as a liaison between members and the company.
Key Responsibilities:
Deliver medical management and health education services to members.
Perform remote and field visits within the Farmington, NM area.
Collect, analyze, and report clinical data for compliance and regulatory use.
Communicate effectively with members, physicians, internal teams, and external resources.
Support care coordination and facilitate access to appropriate health services.
Represent the company professionally in interactions with members and community partners.
Qualifications & Skills:
Registered Nurse (RN) with 3 years of direct clinical care, or
Licensed Professional Counselor (LPC), or
Licensed Master Social Worker (LMSW) with 2 years of clinical practice
Current, valid, and unrestricted license in the state of operations (or eligibility for reciprocity).
Compact license holders must obtain state-specific RN licensure within 90 days if relocating.
3+ years of experience in wellness or managed care settings.
Strong knowledge of health and wellness trends and the healthcare marketplace.
Excellent verbal and written communication skills.
Analytical skills, including experience with medical data.
Proficiency in Microsoft Word, Excel, PowerPoint, databases, and web-based tools.
Valid driver's license, reliable transportation, and current auto insurance.
Ability and willingness to travel within assigned territory for field visits.
