Location: Tallahassee, FL 32301 - Remote + Field Visits (75% travel locally within the region)
Duration: 3+ month(s) with possibility of extension
Schedule: Monday–Friday, standard business hours
Training: Remote via Microsoft Teams (1–2 weeks)
Position Overview:
We are seeking a tech-savvy, detail-oriented, and self-motivated Case Management Coordinator to join our Medicaid Long Term Care Case Management Team. This position involves coordinating care for supportive and medically complex members through both telephonic and in-person visits (home, assisted living facilities, nursing homes). The ideal candidate is comfortable working independently, excels at prioritization, and thrives in a fast-paced, member-centered environment.

Key Responsibilities:
- Manage a caseload of Medicaid Long Term Care/Comprehensive Program enrollees.
- Perform member assessments and develop personalized care plans.
- Conduct field visits (~75% travel) to members’ homes or facilities.
- Coordinate services: PCPs, specialists, prior authorizations, community resources, etc.
- Educate and empower members to make informed healthcare decisions.
- Monitor and document member progress, using case management tools and regulatory guidelines.
- Conduct interdisciplinary case reviews and escalate quality concerns when needed.
- Use motivational interviewing techniques to support lifestyle changes and wellness.
- Maintain detailed and compliant documentation of all interactions.
Required Experience & Skills:
- Case management experience required
- Proficiency in Microsoft Office, including Excel
- Strong verbal and written communication skills
- Ability to prioritize, multitask, and adapt quickly
- Exceptional organizational skills and attention to detail
Preferred Qualifications:
- Bilingual (English/Spanish) strongly preferred
- Experience in Long Term Care or Medicaid case management
Education Requirements:
- Bachelor’s degree required
- Nursing degrees (RNs) are not eligible
- Preferred majors: Social Work, Psychology, Human Services, or a related field

- Candidates must reside in Miami-Dade County (zip codes: 33172 or 33182)
- A valid driver’s license and reliable transportation are required
- Mileage reimbursement will be provided for travel
