Duration: 5 - 6 + month(s) with possibility of extension

Schedule: Full-time (Remote with Travel Required)
Position Overview:
We are seeking a Registered Nurse (RN), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) to join our team as a Field Case Manager. This role involves conducting medical management and health education programs for members enrolled in government healthcare programs. The position includes remote work with regular field visits and responsibilities related to regulatory reporting, member education, and clinical support.

Key Responsibilities:
- Conduct wellness and case management outreach to members.
- Gather, analyze, and report data for regulatory compliance.
- Engage directly with members via phone and in-person field visits.
- Educate members on conditions, treatment plans, and resources.
- Serve as a company representative to members, providers, and community partners.
- Support transition of care, complex case management, and preventive services.
- Document all activities in accordance with regulatory and internal guidelines.
Required Qualifications:
- Active, unrestricted license in the state of Illinois (RN, LPC, or LMSW).
- 3+ years of direct clinical experience (for RN); 2+ years for LPC/LMSW.
- Experience in wellness or managed care, with clinical issue resolution.
- Strong communication and interpersonal skills, both verbal and written.
- Ability to work independently and travel locally as needed.
- Driver’s license, reliable transportation, and current auto insurance.
- Proficiency with Microsoft Office (Word, Excel, PowerPoint) and web-based applications.
Preferred Qualifications:
- 3+ years of clinical experience in a healthcare setting.
- Patient education or condition management background.
- Bilingual in English and Spanish.
- Experience in transition of care or complex case management.
- Certification in Case Management, Project Management, or other relevant fields.
Notes:
- Candidates must reside in or near Danville, Tuscola, Urbana, or Springfield, IL.
- Travel required within the assigned territory (reimbursed mileage).
