Job Purpose
The Health Coordinator is responsible for conducting medical management and health education programs for members enrolled in government health care programs. This role includes gathering, analyzing, and reporting medical management data for regulatory compliance. The coordinator will serve as the company representative to members, ensuring quality care and education.
Key Responsibilities
Conduct medical management and health education programs for members.
Gather, analyze, and submit data for regulatory reports.
Serve as primary point of contact for members and community health resources.
Collaborate with physicians, providers, and internal teams on member care plans.
Facilitate transitions of care and coordinate follow-up services.
Maintain accurate documentation in internal systems.
Required Qualifications
Active RN license (unrestricted) OR LPC/LMSW license in the state of operation.
RN: 3 years of direct clinical care experience, OR
LPC/LMSW: 2 years of clinical practice to obtain license + 3 years wellness/managed care experience.
Strong knowledge of the health & wellness marketplace and managed care programs.
Excellent verbal and written communication skills.
Experience presenting clinical issues to members, physicians, and leadership.
Analytical skills for medical data analysis and reporting
Valid driver’s license, reliable transportation, and ability to travel within assigned territory
PC proficiency: Word, Excel, PowerPoint, and web-based applications.
Preferred Qualifications
3+ years clinical experience (preferably in case management or wellness).
Patient education or Condition Management experience.
Bilingual (English/Spanish) strongly preferred.
Experience managing complex or catastrophic cases.
Certification in Case Management, Training, Project Management, or related field.
