Shift: Monday–Friday | 8:00 AM – 4:30 PM | 40 Hours Weekly
Duration: 13 Weeks (Possible Extension)
Job Summary
We are seeking an experienced Registered Dietitian to support and lead nutrition services in collaboration with the Interdisciplinary Team (IDT). The Registered Dietitian will provide nutrition assessments, counseling, meal planning oversight, and ensure compliance with all dietary and regulatory standards. This role plays a key part in improving participant outcomes while supporting a collaborative, patient-centered care environment focused on vulnerable senior populations.
Key Responsibilities
- Conduct comprehensive nutrition assessments to identify participants’ nutritional needs, preferences, and goals.
- Participate actively in IDT meetings and contribute to individualized care plans.
- Deliver and document nutrition interventions in accordance with physician orders and participant care plans.
- Implement and monitor special diets, meal intake, and nutrition counseling for participants and families.
- Oversee menu planning and meal delivery services to ensure regulatory compliance and dietary accuracy.
- Coordinate nutritional supplements as ordered by providers.
- Provide training and education to day center staff regarding meal delivery and nutrition services.
- Collaborate with physicians and the care team to support smooth transitions between care settings, including hospitals, skilled nursing facilities, and home care.
- Support participant intake and enrollment processes to ensure timely care coordination.
- Monitor participant condition changes and communicate updates promptly to the IDT.
- Complete documentation, trainings, and health screenings accurately and on time.
- Maintain confidentiality and comply with all organizational policies and quality improvement initiatives.
- Adapt to changing business and participant care needs as required.
Required Qualifications
- Registered Dietitian credential required.
- Minimum 4 years of experience managing nutrition services in a similar healthcare setting.
- At least 1 year of experience working with frail or elderly populations.
- Strong organizational, communication, and interpersonal skills.
- Ability to work independently in a fast-paced environment.
- Experience mentoring and supporting team members.
- Demonstrated cultural competency and ability to work with diverse populations.
- Commitment to delivering compassionate, participant-centered care.
Additional Requirements
- Must be legally authorized to work in the job location.
- Ability to work flexible schedules if needed, including evenings, weekends, or holidays.
- Must perform duties within professional scope and organizational guidelines.
