Job Title: Patient Care Coordinator | Case Manager
Locations: Clackamas, OR 97015
Duration: 3 Months+ (Possible Extension)

Shift: 10x4 Days or nights/12.00 AM – 12.00 PM or 12.00 PM – 12.00 AM

Job Responsibilities:
  • Responsible for assessing client needs, developing care plans, coordinating services, and advocating for clients to ensure they receive appropriate care and support. 
  • Strong communication and interpersonal skills.
  • Ability to build rapport and trust with clients and their families.
  • Excellent organizational and time management skills.
  • Problem-solving and critical thinking abilities.
  • Familiarity with community resources and support networks.
  • Knowledge of case management principles and practices.
  • Ability to work independently and as part of a team.
  • Proficiency in relevant software and documentation systems.
  • Delegates tasks to support staff.
  • Performs other duties as assigned.
 
Required Qualifications:
  • 1-year recent (within the last 3 years) full-time equivalent experience
 
Preferred Qualifications:
  1. Bachelor’s degree in nursing or related field.
 
Requirements:
  • Compact License
  • BLS/ ACLS Certification
