Job Title: Healthcare Consultant I
Location: Central/SE Ohio – Candidates must reside in one of these counties: Ashland, Richland, Wayne, Fayette, Highland, Ross, Pike, Jackson, Licking, Muskingum, Coshocton, Fairfield, Franklin, Morrow, Washington
Note: Occasional travel to corporate office in Franklin County may be required (1-2 times/year).
Duration: 6 Months+ (Possible Extension)
Pay Rate: $27.16/hr. on W2
Work Type: Field-based telework with travel within the assigned region up to 50% or more of the time
Position Summary:
The Case Management Coordinator provides comprehensive care coordination and healthcare management for children and youth enrolled in the Client Program. This role ensures appropriate healthcare outcomes by collaborating with members, families, and multidisciplinary teams while supporting individualized care plans and addressing complex behavioral health needs.

Position Summary:
The Case Management Coordinator provides comprehensive care coordination and healthcare management for children and youth enrolled in the Client Program. This role ensures appropriate healthcare outcomes by collaborating with members, families, and multidisciplinary teams while supporting individualized care plans and addressing complex behavioral health needs.
Key Responsibilities:
Member Evaluation & Care Coordination:
- Conduct comprehensive evaluations of referred members’ needs, benefits, and available services using care management tools.
- Identify high-risk factors and service needs impacting member outcomes.
- Coordinate and implement assigned care plan activities, monitoring progress and making adjustments as necessary.
- Serve as a single point of contact for members and assist in resolving immediate gaps in care and access.
- Consult with case managers, supervisors, medical directors, and other programs to overcome barriers and achieve optimal outcomes.
- Present cases in case conferences for multidisciplinary review.
- Collaborate with members’ Child and Family Teams.
- Escalate quality of care issues through established channels.
- Use motivational interviewing and coaching skills to promote lifestyle and behavioral health improvements.
- Empower members to actively participate in healthcare decision-making.
- Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines.
- Identify continuous quality improvement and compliance opportunities.
- Document all interventions and outcomes accurately and timely.
- Must be willing and able to travel within assigned region up to 50% of the time.
- May need to work beyond core business hours to meet member needs.
- Reliable transportation required; mileage reimbursed per company policy.
Required Qualifications:
- Bachelor’s degree or non-licensed master-level clinician in behavioral health, human services, health services, or public health (e.g., psychology, social work, marriage and family therapy, counseling, juvenile justice).
- Minimum 2 years of experience in behavioral health, social services, or human services.
- Minimum 2 years of experience with personal computers, multi-system navigation, and MS Office Suite (Outlook, Word, Excel, SharePoint).
- Minimum 2 years providing community-based services to children/youth and families in areas such as children’s mental health, child welfare, developmental disabilities, or juvenile justice.
- Experience in one or more of the following areas: family systems, community systems/resources, case management, child and family counseling/therapy, child protection, child development.
- Knowledge of Ohio delivery systems, including local community networks and resources.
Preferred Qualifications:
- Case management and discharge planning experience.
- Managed care experience.
- Medicaid program experience.
Core Competencies:
- Clinically and culturally competent in managing complex cases.
- Strong critical thinking, problem-solving, and negotiation skills.
- Ability to educate, coach, and empower members and families.
- Effective communication and interpersonal skills for collaboration with internal teams, providers, and members.
- Strong organizational skills and ability to manage multiple priorities.
