Job Summary:
- The position is also responsible for outreaching members to complete health risk screenings and providing coverage for any additional outreach efforts that are needed.
- Telephonic, Work from home position in Chicagoland area in case they need to go onsite for a meeting (seldom).
Education and Certifications:
- Bachelor Degree OR 4 years’ health care, social services or community advocacy experience.
- Community Health Worker (CHW) certification from Client approved college.
Skills and Experience:
- Knowledge of medical terminology.
- Experience coordinating member related needs, providing assistance to members, and analyzing member needs.
- PC proficiency including Microsoft Office applications.
- Customer service skills.
