Schedule:

• Day shift
• 36 guaranteed hours per week
• Weekend hours as needed
• No on-call required
• Float between both locations

 
Role Overview:
Provide daily skilled physical therapy treatment to residents in long-term care and skilled rehab. Typical patients include elderly residents with acute-on-chronic conditions such as CHF, COPD, arthritis, fractures/falls, neurological issues, and general weakness.
 
Patient Ratios: 1:8 (therapy)
 
Equipment & EMR:

• Equipment provided
• EMR systems: Optima and PointClickCare

 
Required Education & Certifications:

• Licensed Physical Therapy Assistant (PTA)
• CPR

 
Experience:

• Minimum 2 years in skilled/LTC/geriatrics

 
Must-Have Skills:

• Skilled in LTC and geriatrics therapy
• Comfortable treating patients with multiple chronic and acute conditions

 
Preferred Skills:

• Experience with elderly, long-term care, and short-stay rehab residents

 
Personality Fit:

• Flexible, easy-going, team player

• Scrubs (any color) or khakis with a collared shirt
