Job Duties:
1. Social Support & Navigation:
- Conduct social needs screening/assessment for primary care patients.
- Address social determinants of health (SDOH) such as food, housing, and transportation insecurity.
- Maintain a resource list of SDOH services with eligibility, referral process, and contact information.
- Collaborate with primary care nurses, providers, and other social workers to coordinate resources.
- Work with hospitals for discharge planning, follow-up, and patient education.
- Assist with obtaining patient medical records and securing needed medical equipment.
- Conduct follow-ups on care plans.
- Identify patients lost to follow-up or overdue for care and assist them in returning to care.
- Schedule, coordinate, and track specialist and imaging referrals.
- Maintain resource lists for specialty referrals including eligibility, costs, and contacts.
- Assist with obtaining patient records from specialists and imaging centers.
- Help patients access low-cost prescription options such as patient assistance programs or discount retailers.
- Assist with program applications (e.g., CoverRx, RxOutreach) and act as liaison with drug companies.
- Document patient interactions, updates, and consults in the patient record.
- Provide patient education or identify appropriate education resources.
- Participate in onboarding, orientation, regional office, and clinical meetings.
- Attend provider meetings, Health Councils, and community meetings to build relationships and promote health department services.
- Identify barriers to care and work toward solutions.
