Managed Care Community Connector
Job Title: Managed Care Community Connector
Must reside in one of the 5 boroughs of New York (Queens, Brooklyn, Staten Island, Manhattan, Bronx)Location:
Pay Rate: $27.92/hr. (W2)
Monday – Friday, 8:30 AM – 5:00 PM ESTSchedule:
Travel:Travel Member Homes and get consent forms signed, must have reliable transportation
Position Overview
The Community Connector serves as a field-based advocate, working directly with members to ensure access to healthcare and community resources. This role involves retrieving consent forms, assisting members with navigating benefits, and supporting care coordination in collaboration with healthcare services teams.
Day-to-Day Responsibilities
Travel to members’ homes to obtain signed consent forms.
Meet with members in homes, nursing facilities, shelters, or provider offices.
Assist with scheduling provider appointments, arranging transportation, and medication needs.
Support members in accessing social services (housing, food, employment, etc.).
Conduct outreach to locate and re-engage disconnected members.
Research member whereabouts using internal systems and community resources.
Help members maintain Medicaid eligibility and connect with financial resources.
Participate in outreach initiatives requiring both phone and in-person contact.
Must-Have Skills & Qualifications
Language: Must be Spanish-speaking.
Education: HS Diploma/GED required.
Experience: Minimum 1 year working with underserved or special needs populations.
Technical: Basic computer skills.
Licensing/Travel:
Active, valid driver’s license (no restrictions).
Good driving record.
Reliable transportation.
Comfortable with extensive travel within the 5 boroughs.
