Duration: 3 Months+ (Possible Extension) Contract

Shift: 08:00 AM – 06:30 PM (Days)

Minimum Requirements:

• Active RN Oregon License & BLS
• 1 year of current experience as RN Case Manager (Acute/Hospital)
• Skills: Coordinating patient care, scheduling, or supporting clinical teams preferred

 
Summary:
Supports patient flow, communication, and coordination activities to ensure timely, accurate, and highquality care delivery.
 
Responsibilities:

• Coordinate patient scheduling, messages, records, and followup activities to support efficient care delivery.
• Communicate with patients, providers, and staff to ensure accurate information exchange and smooth transitions.
• Maintain documentation, assist with regulatory compliance needs, and support daily operational workflow
