Position Summary:
- The Care Management Associate provides support for the coordination of healthcare services by assisting in care team intake, screening, and implementation of care plans.
- The role promotes effective utilization of healthcare services and supports the quality and efficiency of care delivery.
- Perform initial triage and review of Care Team tasks.
- Identify reasons for admission, facilities, and member information to apply correct assessment tools.
- Refer members to appropriate programs such as Case Management, Disease Management, or Specialty Programs.
- Must be a Kentucky resident
- Must be able to travel to Louisville occasionally for work functions (5% 10%)
- High School Diploma or GED
- 2 4 years of experience as a medical assistant, office assistant, or similar role
- Familiarity with Foster Care, juvenile justice, adoption assistance, and behavioral health services
