Location: Stockton, CA -95204
Assignment Length: 3–6 Months
Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Pay Range: $50 - $55/hr. on W2

Key Responsibilities:
- Conduct comprehensive nutrition assessments to identify participant needs, preferences, and goals
- Actively participate in IDT meetings and contribute to individualized care plans
- Deliver and document nutrition interventions per physician orders and care plans, including special diets and nutrition counseling for participants and families
- Oversee menu planning and meal delivery to ensure regulatory compliance, food safety, and accurate diet implementation
- Coordinate and monitor nutritional supplements as ordered
- Provide training and education to day center staff related to meal service and nutrition programs
- Collaborate with physicians and IDT members to support care transitions across settings (hospital, SNF, home, etc.)
- Support intake and enrollment processes to ensure timely and effective participant onboarding
Required Qualifications:
- Minimum 4 years of experience managing nutrition services in a similar healthcare setting
- At least 1 year of experience working with a frail or elderly population
- Strong organizational, communication, and prioritization skills
Education & Certifications:
- Bachelor’s Degree (Required)
- Registered Dietitian (RD) credential via the Commission on Dietetic Registration (CDR)
- Basic Life Support (BLS) Certification
