Job Location: New York, NY 10029
Job Duration: 3+ months (with possible extension)
Shift: Day shift, 8:30 am to 4:30 pm
Pay Rate: $60 - $65/hr. on W2

Responsibilities:
- Member Assessment & Support: Assess members' clinical, psychosocial, financial, and environmental needs. Address barriers to care and optimize resources to improve their healthcare experience.
- Engagement & Empowerment: Build collaborative relationships with members and caregivers, empowering them to manage their health and work toward sustainable well-being. Engage members in transitional periods, especially from shelter to permanent housing.
- Develop Care Plans: Create comprehensive care plans tailored to each member’s unique needs, including input from the member, caregivers, and healthcare providers.
- Health Education & Advocacy: Provide education on chronic disease management, preventive care, medication adherence, and home safety. Advocate for members by assisting them in navigating healthcare and social support systems.
- Collaborative Care Management: Work closely with clinical providers, behavioral health professionals, and community-based services to ensure coordinated care and resolve barriers to treatment.
- Case Management: Ensure continuity of care by monitoring risks, gaps in services, and care needs. Use data to track outcomes and adjust care plans as necessary.
- Documentation & Compliance: Maintain accurate and up-to-date documentation in compliance with organizational policies and regulatory standards.
Required Skills & Experience:
- Inpatient medicine, surgery, (med/surg), rehab experience
- Manager would like someone with Hospital experience
- At least 1 year of experience
Required Education and License:
- Master’s degree in social work
- Valid Licensed Master of Social Work (LMSW) license issued by NYSED required
