Job Title: CNA – Long Term Care

Location: Brooklyn, NY 11212
Duration: February 10, 2026 – May 9, 2026
Schedule: 5 days per week, 7.5 hours per day (37.5 hours/week)
Shifts: Day (7A–3P), Evening (3P–11P), Night (11P–7A)
Weekend Requirement: Every other weekend
Local Requirement: Must be within 50 miles
Pay Range: $23-$26/hr.




Required Skills & Experience

  • Knowledge of prevention of skin breakdown

  • Ability to communicate with residents in a compassionate and gentle manner

  • Ability to understand and carry out verbal and written instructions



Preferred Skills & Experience

  • Long Term Care experience preferred




Education Requirements

  • High School Diploma or GED

  • Completion of at least 100 hours from a New York State–approved Residential Health Care Facility Nursing Assistant Training Program




Required Certifications & Licensure

  • New York Nurse Aide Certification

  • Must be listed on the New York State RHCF Nurse Aide Registry




Job Summary

Provides direct patient care and support to residents in a long-term care setting, assisting with daily living activities, personal hygiene, mobility, nutrition, monitoring, documentation, and overall resident comfort while maintaining safety, dignity, and quality of care.




Duties & Responsibilities

  • Provide complete or partial baths, scheduled showers (minimum twice weekly), shampooing, grooming, shaving, nail care, oral hygiene, and denture care

  • Assist with dressing, undressing, toileting, incontinent care, skin care, and range of motion exercises

  • Encourage resident participation in daily activities and ambulation programs

  • Assist with transfers using mechanical lifts with mandatory two-person assistance and buddy system

  • Turn and reposition residents every 2–3 hours to prevent skin injury and contractures

  • Transport residents to clinics, activities, and offsite appointments during working hours

  • Monitor bowel and bladder programs and document results in NAAARS

  • Measure and record intake/output, meal consumption, weights, and vital signs in the EMR

  • Observe and promptly report changes in resident condition or behavior to nursing staff

  • Assist with admissions, transfers, discharges, and dressing changes as directed

  • Answer call bells and alarms promptly and maintain a restraint-free environment

  • Maintain clean, safe, and clutter-free resident rooms and service areas

  • Document care accurately and provide shift reports to nurse-in-charge

  • Monitor residents every 15 and 30 minutes as assigned

  • Perform other duties as assigned by nursing leadership and assist during emergencies as needed
