Position Summary
The Case Management Coordinator facilitates appropriate healthcare outcomes for members by:

• Conducting comprehensive member evaluations
• Coordinating care plans
• Providing education and resources
• Partnering with members, families, providers, and community resources

Required Qualifications

• Education: Bachelor s degree or non-licensed master s degree in behavioral health, human services, health services, or public health (e.g., psychology, social work, marriage and family therapy, counseling, juvenile justice).
• Experience:
  • 2+ years in behavioral health, social services, or human services.
  • 2+ years working with children and families in mental health, child welfare, developmental disabilities, juvenile justice, or public sector/community-based settings.

Preferred Qualifications

• Experience in case management and discharge planning
• Managed care experience
• Medicaid experience
