Position Summary:

The Intake Services Coordinator, under clinical supervision of a licensed social worker or licensed professional counselor, coordinates the referral development and pre-admission patient assessment process.

Essential Job Duties/Responsibilities

Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW.

Facilitates groups, family and individual sessions and completing all necessary documentation.

Schedules Unit assessments, meetings and appointments as appropriate.

Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.

Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.

20%

Reviews newly admitted patient accounts for eligibility and authorization daily.

10%

Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.

Participates as an active member of the interdisciplinary team.

5%

Addresses issues in a timely manner including crisis and higher level of care referrals.
