Registered dietician LTACH to provide initial and reassessments on all patients admitted to the hospital. Ensure proper completion of documentation of assessments, care plans, education and discharge planning. Interviews patients, family members and staff to obtain diet history, food preferences needed to develop and or update plan of care. Coordinates with Culinary Service Manager to resolve patient's concerns or dissatisfaction with diet or meal service.
Facility Requirements:
Must have BLS certification must be attached to submission packet (Must be through the American Heart Association); RQI certifications will not be accepted
Must be a Registered Dietician with the Commission on Dietetic Registration, this must be attached to the submission
Must have KY Dietician License; this needs to be attached to the submission
Must have 2 years recent Dietician in an Acute Care Hospital setting experience
Shifts will be Monday - Friday 8a-4:30p, no weekend or on-call
