=========
Consistently performs initial patient evaluations, including administering appropriate tests/measures, identifying impairments and functional limitations, and establishing initial plan of treatment in conjunction with patient/caregiver. Always selects and administers treatment strategies based on current physician orders and established treatment plan. Proactively identifies functional improvement and goal attainment on a regular basis, supported by documented subjective and objective changes. Always updates treatment plans and goals per department standard. Consistently completes documentation following department standard format and timeframes. Documentation reflects skilled intervention and supports therapeutic intervention as reasonable and necessary. Consistently achieves productivity per department standards and completes job duties in an efficient and effective manner. Always takes initiative to use non-productive time to benefit the department. Home Health Care: Consistently complies with applicable rules and regulations, demonstrates accuracy with OASIS, adheres to National Patient Safety Goals, demonstrates responsible utilization of resources for each patient and participates in department PI activities.

Education

Master s degree in Communication Disorders required. Certificate of clinical competence from the American Speech/Language Hearing Association or adequate course work and experience to qualify for Clinical Fellowship Year required. Wisconsin licensure or eligible. If eligible, must receive license within six months of hire required. None, but two years of clinical experience in adults, pediatrics, or developmentally disabled preferred. CPR certification and valid WI driver license required.
