Job Title: Social Worker II
Location: Oklahoma City, OK 73104
Duration: 3 + Months (Possibilities of Extension) , 40hrs/week
Shift: Full-Time | Monday–Friday | 8-hour shifts | 40 hours/week
Job Description:
Under general supervision, the Social Worker II provides professional social services to patients and their families.
This includes psychosocial assessments, crisis interventions, discharge planning, and advocacy to support optimal patient care and outcomes.
Essential Responsibilities:
- Provide social services to patients and families within an assigned medical service area.
- Conduct discharge planning evaluations and psychosocial assessments.
- Communicate findings and concerns to the healthcare team.
- Offer counseling related to disease, treatment, financial or social challenges, and emotional stress.
- Identify high-risk social cases and provide crisis intervention (e.g., child abuse/neglect, physical abuse).
- Serve as a liaison between patients/families, hospital staff, and external agencies.
- Evaluate and adjust social work services to achieve desired patient outcomes.
- Assist with discharge planning, placement, and referrals to post-acute or community-based services.
- Support patients and families in managing difficult diagnoses, treatment transitions, or functional changes.
- Advocate for patient rights, informed decision-making, and ethical care practices.
- May collect data related to referrals, extended stays, or service utilization.
- May supervise students or new employees as assigned.
General Responsibilities:
- Perform other related duties as assigned.
- Master’s Degree in Social Work (MSW) from an accredited school.
- Licensed Master Social Worker (LMSW) strongly preferred.
Experience:
Hospital or healthcare setting experience preferred.
