Shift: 8:00 PM – 8:30 AM
Schedule: 3 Days Per Week, 36.00 Hours Per Week
Schedule Notes: Thursday, Friday, Saturday – 8:00 PM to 8:30 AM
Pay Range: $30-$35/hr.
Required Skills & Experience
-
Must be able to observe and report on patients’ physical, mental, and social conditions
-
Experience assisting patients with daily living and personal hygiene
-
Ability to implement therapeutic and group activities as part of treatment programs
-
Able to take and enter patient vital signs and perform finger stick tests (post-training)
-
Must maintain cleanliness and order of assigned unit areas
-
Must disinfect personal items using approved disinfectants
-
Must be able to escort patients internally and externally as directed
Preferred Skills & Experience
-
CNA certification or CNA background – Preferred
Education
-
High school diploma or equivalent approved by a State Department of Education or recognized accrediting organization – Required
Licensure & Certifications
-
Valid and current certification in Basic Life Support (BLS) through the American Heart Association – Required

Completion of Client training in the application/use of restraints and seclusion – Required
-
Completion of Client training in performance of vital signs and grooming (within 90 days of appointment) – Required
Job Summary
-
In this voluntary program, Client will offer its guests onsite ambulatory behavioral health treatment and comprehensive support in a home-like environment.

The program aims to keep individuals discharged from the hospital on a path toward sustained success, with the goal of reducing emergency room visits and inpatient hospitalizations, decreasing street homelessness and reliance on shelters, and ultimately moving into permanent housing.

Program guests will stay in the home-like environment for up to 12 months or until transition to permanent supportive housing and treatment.

Under the supervision of a professional nurse or other assigned staff, the role involves:

Observing and reporting on patients’ physical, mental, and social conditions

Planning and implementing orientation and daily living activities
-
Supporting hygiene care, feeding, and mobility
-
Encouraging participation in individual and group therapeutic activities
-
Taking vital signs and documenting them in EMR
-
Escorting patients internally and externally as directed
-
Maintaining a clean, safe, and therapeutic ward environment

Disinfecting patient items and supporting the team in restraint/seclusion procedures when trained and necessary
