Job Description :
Major Responsibilities
1. Serves as the key external-facing member of cross-functional/divisional team of designated resources assigned to manage chart abstraction for assigned providers and facilities.
2. Informs and educates providers and physicians on Client s chart collection activities and is responsible for fostering provider relationships and adoption of Client Risk Adjustment initiatives
3. Responds to provider inquiries regarding provider related information and data in order to resolve issues and increase chart retrieval volumes
4. Educates, monitors, analyzes, documents and interacts with physician and other health care professionals to ensure cooperation and understanding of chart retrieval projects.
5. Alerts leadership to potential issues and opportunities found through physician contact.
6. Maintains a thorough knowledge of healthcare industry trends and developments through the review of industry publications, newsletters and formal training.
7. Ensures medical records are assembled in standard orders and are accurate and complete
8. Ensure and meet productivity expectations standards for collection of medical records
9. Maintain strict confidentiality and security with all patient records while ensuring compliance to HIPAA standards
10. Receive and process chart request in accordance with the Company Standards. Ensures integrity of chart retrieval database and quality audit controls of all medical records documents in a timely and accurate manner.
Education/Experience
Bachelor's degree or relevant experience in lieu of degree required
Requires a minimum of two (2) years healthcare experience, preferably with provider or client focus/interaction.
Additional Licensing, Certifications, Registrations
Requires a valid New Jersey driver s license and good driving record, to be determined via a New Jersey DMV check.
HIPAA Privacy Certification preferred
Knowledge
Requires knowledge of healthcare industry.
Requires knowledge of New Jersey physician community.
Requires strong PC skills including Microsoft Office
Basic navigation and knowledge of clinical EMRS systems: Allscripts, Epic, E-clinical works, Nextgen are a plus.

Strong interpersonal skills such as diplomacy, patience and politeness
Time management skills
Ability to balance multiple projects
- This position is accountable for physician outreach, relationship management, issue identification and problem resolution related to the release of medical records.
- This is achieved by serving as the primary member of a team made up of people from various functional areas in the end-to-end provider experience process and is responsible to support chart retrieval collection throughout New Jersey and surrounding areas.
- This includes maintaining effective relationships with all facets of the physician community, including physician education.
- This position is responsible to work directly with network physicians for retrieval of medical records electronically or through scanning from provider offices and facilities
