Job Title: Birth Registrar
Location: Houston, TX - 77030 (Onsite)
Duration: 3+ Months (possible extension)
Schedule: M-F & every-other weekend (when working weekends, off on Friday & Monday)

Summary:
Responsibility A: Provides mothers with Mother s Worksheet for Birth Certificate completion.
  • Runs Epic generated Birth Report.
  • Reviews Epic Birth Report to determine patients admitted for delivery.
  • Provides mothers with Mother s Worksheet Form (VS-109.1) for birth certificate completion.
  • Assists the mother with completion of the Mother s Worksheet (VS-109.1) as needed.
  • Completes the Verification of Birth Facts Form (VS-H4 REV 5/05).
  • Reviews the Verification of Birth Facts Form (VS-H4 REV 5/05) with parents and obtains signature on form.

Responsibility B: Informs eligible parents of Texas laws and regulations regarding Acknowledgement of Paternity.
  • Explains the State of Texas laws regarding Acknowledgment of Paternity.
  • Provides Parents with explanation of Rights and Responsibilities of Paternity Form (VS-159.1M).
  • Provides information on the availability of child support services and provides the Establishing Paternity brochure.

Responsibility C: Ensures that Birth Certificates are filed with the State of Texas.
  • Completes State of Texas Certificate of Birth Record (VS-111 Rev 1/05).
  • Enters data into TER (Texas Electronic Registrar) for short form (Legal Record) of birth certificate.
  • Prints Verification of Birth Facts (VS-H4 REV 5/05).
  • Affirms consent in TER (Texas Electronic Registrar).

Responsibility D: Ensures the completion of Acknowledgement of Paternity.
  • Obtains valid ID from both parents and make copy for files.
  • Reviews the Acknowledgment of Paternity Form (VS-159.1M) for accuracy and parents signatures.
  • Provide parents with copy of signed Acknowledgement of Paternity Form (VS-159.1M).

Responsibility E: Maintain accurate record keeping ensuring compliance with State of Texas Laws and regulations.
  • Scans the Verification of Birth Facts Form (VS-H4 REV 5/05) into the appropriate EMR.
  • Scans the Mother s Worksheet Form (VS-109.1) into appropriate EMR.
  • Scans the Acknowledgment of Paternity Form (VS-159.1M) and the Acknowledgement of Paternity Survey into appropriate EMR.
  • Scans the Social Security Letter and Immtrac (Texas Immunization) Registration Forms (ImmTrac NB-2) into appropriate EMR.

Responsibility F: Obtains completed ImmTrac (Texas Immunization) Registration Form and Social Security Form from parent.
  • Provides ImmTrac Registration and Social Security Form (ImmTrac NB-2) to parent.
  • Receives completed ImmTrac Registration and Security Form (ImmTrac NB-W) forms from parent before discharge.

Responsibility G: Receptionist/Front desk coverage.
  • Greets all customers utilizing appropriate customer service skills.
  • Handles routine questions consistently and in a timely fashion.
  • Utilizes appropriate telephone skills.

Responsibility H: Retrieves all medical records needed to complete ROI request.
  • Reviews each request for release of patient information requirements.
  • Retrieves requested patient information.
  • Enters a Quick Disclosure in the EHR to track the patient information requested and released.

Skills:
  • Knowledgeable in the use and analysis of HIM patient information systems applications.
  • Knowledge of medical terminology.
  • Knowledge ofICD9, rCDIO, and CPT coding classification systems and guidelines.
  • Knowledgeable in CMS, Texas Medicaid, and other regulatory agency rules and regulations related to Health Information Management.
  • Effective verbal, written and/or interpersonal communication skills. Expressing oneself verbally in an accurate, understandable, and succinct manner to individuals or groups; using appropriate non-verbal signals to emphasize spoken words; adapting communication style and tone to fit the situation and audience; demonstrating poise during group discussions or formal presentations; holding others' attention; responding clearly and tactfully to questions.
  • Understanding and showing respect and appreciation for the uniqueness of all individuals; leveraging differences in others' perspectives and ideas; appreciating cultural differences and adjusting one's approach to successfully integrate with others who are different from oneself.
  • Intermediate proficiency in basic computer applications such as Microsoft Office, e-mail and internet.
  • Demonstrates an independent work initiative, sound judgment, diplomacy, tact and professional demeanor.
  • Attention to detail, works well in a team environment

Preferred:
  • Patient-facing experience

Education:
  • High School Diploma

Preferred:
Associates Degree, required AOP CERT - Acknowledge of Paternity Cert Standard ADA Settings required within 6 months of hire/transfer (Issued by Attorney General of Texas)

Preferred:
  • RHIA - Cert-Reg Health Inform. Admins American Health Information Management
  • Association (AHIMA) or RHIT - Cert-Reg Health Inform. TECH American Health Information Management
  • Association (AHIMA); required 3 years records management or medical records experience and 1 year experience in in birth registry;
  • An Associate's or a Bachelor's degree may substitute for this experience requirement.
