Job Title: Master of Social Work (MSW) – (PCLS)
Location: Moreno Valley, CA 92555
Duration: 3+ Months (Possible extension)
Schedule: Mon, Thur, Fri &Sat | 8:30AM – 7:00PM (10hr/Day |40 hrs/week)
Pay: $55/hr on W2

Core Responsibilities

• Conduct comprehensive psychosocial and mental health assessments for patients on legal holds (5150/5585, 5250, TCON, PCON).
• Complete Initial Assessments and Social Determinants of Health (SDOH) documentation within EPIC.
• Evaluate mental status, housing, support systems, substance use, financial factors, and discharge needs.
• Collaborate closely with psychiatrists, nursing leadership, and care management on treatment and transition plans.
• Coordinate psychiatric placements, outpatient follow-ups, and community resources.
• Maintain communication with families and external agencies to support safe, timely discharges.
• Monitor legal hold timelines, conservatorship potential, and status transitions.
• Provide crisis intervention and manage high-acuity behavioral health cases with professionalism and composure.

Required Qualifications

• Master’s Degree in Social Work required.
• Certification: ACSW, APCC, AMFT Intern #, or LCSW/LMFT/LPCC. (Licensure not mandatory.)
• Minimum 2 years of clinical experience.
• Mental health crisis intervention experience strongly preferred.
• Prior counseling background is a plus.
