Job Title: Clinical Pharmacist
Job Location: Dallas, TX ( Remote role)
Job Duration: 12 Months (possibility of extension)

Position Type:

  • This will be a 12 month role, that might extend - hours will be based on business needs.
  • Flexible scheduling - HM will consider candidates who only wants part time work.

Job Summary:

  • The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines.
  • The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application.
  • The data to be transcribed will be either hard-copy or available from a client Health application.
  • The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients.
  • The pharmacist is a member of the MTM product team.

Essential Job Responsibilities:

  • Conducting outbound phone calls. (100 calls per day).
  • Completing comprehensive medication reviews.
  • Conducting medication therapy management services.
  • Accuracy and speed in entry of patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application.

Additional Job Responsibilities:

  • Attend training sessions to acquire/enhance skills related to Essential Job Responsibility.

Education/Licensing Requirements:

  • Valid Pharmacist License in Florida - please screenshot and put at top of resume.

Experience Requirements:

  • At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred).
  • Familiar with driving Part D Star measures.
  • Specifically, SUPD, SPC, and CMR.
  • Understanding of pharmaceutical and medical vocabulary.
  • Clinical experience in medication review.

Essential Skills/Experience:

  • Fluently speak, read, and write English.
  • Excellent verbal and written communication skills.
  • Experience using salesforce preferred.
  • Must be tech savvy.

  • Discrete/ability to maintain confidentiality.
  • Detail-oriented.
  • Sense of urgency.
