Job Title: Care Support
Locations: Remote (Must reside in California)
Duration: 3 Months+ (Possible Extension)
Shift: Start time: 8:00 AM or 9:00 AM
Pay Range: $25/hr. on W2
Job Description:
This position is responsible for providing administrative and logistics support to Clinical Operations Care Managers in the execution of care planning and coordination of care and services associated with person-centered care plans for the Medicaid, Medicare, and Whole Health markets.
This is an unlicensed member of the care team. Enables community research and education for members in care management, along with supporting member engagement with their primary care and other sources of support. Works with licensed Care Managers to meet standards of care and performance set by the Member Market Leads and Care Management Center of Operational Excellence Lead.
Key Responsibilities:
- Provides non-clinical support to Care Managers by offering administrative and logistics support for Care Manager’s member caseload in the Care Management continuum.
- Performs administrative tasks for the development and implementation of care plans, including outreach to providers and community resources to connect members and perform appropriate follow-up.
- Provides information to members and providers regarding mental health, physical health, and/or substance abuse benefits, community treatment resources, managed care programs, and company policies and procedures as appropriate.
- Performs research on the availability of community resources and services and supports efforts to link members to appropriate services.
- Troubleshoots and communicates with appropriate care team members or Care Managers when an appropriate provider, type of service, or appropriate level of care cannot be readily found.
- Participates in the treatment team and assists in coordinating physical, psychological, psychiatric appointments and services for members.
- Works collaboratively with all clinical operations team members including other care support resources, care managers, transition of care teams, etc., to enable effective and efficient support of member care.
- Maintains professional relationships with external stakeholders, such as caregivers, providers, community services, etc.
- Demonstrates flexibility in areas such as job duties and schedule in order to aid in better serving members and help the company achieve its business and operational goals.
- Assumes responsibility for self-development and career progression, including staying updated on ever-changing information to ensure accuracy when dealing with members and providers.
- Proactively seeks and participates in ongoing training (formal and informal) in all aspects of the Care Support role and Client-required training.
- Assists in the mentoring and training of new Care Support team members.
- Coordinates and supports the distribution of correspondence and materials to members and providers as required.
- Participates in activities to support efficient and effective management of operations, including data collection, tracking, and analysis.

Experience Required:
