Clinical Pharmacist

Serves as a clinical resource supporting pharmacy quality measures, with a focus on Star/HEDIS programs. Responsible for developing, managing, and executing initiatives to achieve 5-star performance. Acts as a liaison between the pharmacy department, health systems, vendors, and stakeholders to drive clinical and financial outcomes.

• Manage outreach and performance for Star/HEDIS clinical programs (e.g., adherence, SUPD, MTM/CMR)
• Oversee daily activities to ensure clinical goals and quality metrics are met
• Identify gaps and implement clinical solutions to improve care and reduce costs
• Serve as liaison between pharmacy teams and health systems to drive program adoption
• Develop and monitor clinical programs, therapeutic initiatives, and care pathways
• Track, analyze, and report program performance and patient outcomes
• Collaborate with data teams to monitor quality, cost, and Medicare trends
• Conduct provider, member, and pharmacy outreach as needed
• Ensure alignment of clinical programs with organizational and financial goals

• Bachelors or PharmD in Pharmacy
• 25 years of relevant experience
• Experience in health plans, Medicare, retail pharmacy, or clinical programs
• Knowledge of CMS Star/HEDIS measures
• Current Pharmacist license in New Jersey
