Job Title: Medical Social Worker II (LCSW) – Inpatient Psych
Location: Fremont, CA
Contract Type: 13 weeks (Strong potential for extension)
Schedule: Sunday – Thursday | 8:30 AM – 5:00 PM
Pay Rate: $70/hr – $90/hr
Position Summary:
We’re urgently seeking a Medical Social Worker II with a California LCSW license and inpatient psychiatric experience to join our multidisciplinary care team in Fremont, CA. This role is mission-critical in delivering psychosocial support and ensuring continuity of care for complex patient populations.
Key Responsibilities:
- Perform comprehensive psychosocial assessments for patients and families in an inpatient psych setting.
- Provide counseling and emotional support for patients navigating trauma, illness, or end-of-life challenges.
- Design and coordinate individualized discharge plans in collaboration with physicians, RNs, and case managers.
- Deliver crisis intervention during acute psychiatric or emergency situations.
- Educate patients/families on community resources, financial aid, and support programs.
- Advocate for patient rights, including support with advance directives and ethical decision-making.
- Attend and contribute to interdisciplinary rounds, case conferences, and care team meetings.
- Maintain accurate and timely documentation in accordance with HIPAA and facility protocols.
- Actively address barriers to care such as housing, transportation, and social support.
- Participate in quality improvement projects to enhance department efficiency and patient care.
- Demonstrate cultural competence in working with diverse patient populations.
- Stay current with CE requirements and licensure compliance.
Requirements:
- Active LCSW License in California (Required)
- Minimum 1 years of Inpatient Psychiatric Experience
- Strong understanding of discharge planning, psychosocial assessment, and crisis management
- Excellent interpersonal, documentation, and organizational skills
- Familiarity with HIPAA guidelines and multidisciplinary collaboration practices
