Job Title: Care Manager I
Location: Springfield or Bloomington, IL 62705 (Local travel required)
Duration: Up to 4 months (potential for extension not guaranteed)
Schedule: Monday – Friday, 8:30 AM – 5:00 PM CST
Pay Rate: $41.29/hr. on W2
Work Environment: Hybrid – Primarily field-based with flexibility to manage own schedule
Job Summary:
- The Case Manager is responsible for the health care management and coordination of members receiving waiver services, with a focus on optimizing clinical, financial, and quality of life outcomes.
- This role involves direct engagement with members, including face-to-face home visits, as well as coordination with providers and support systems.
Key Responsibilities:
- Conduct monthly face-to-face home visits and assessments with assigned members.
- Manage and coordinate care plans tailored to members’ needs and goals.
- Communicate regularly with providers, caregivers, and community resources.
- Document all case management activities in the client's electronic system.
- Monitor, evaluate, and modify care plans to ensure optimal outcomes.
- Maintain compliance with federal and state guidelines, as well as company policies.
- Attend team meetings and contribute to Interdisciplinary Care Team (ICT) discussions.
- Support onboarding of new team members when appropriate.
- Maintain professional relationships with all stakeholders.
Day-to-Day Duties:
- Completing documentation and paperwork.
- Responding to emails and calls from members and providers.
- Coordinating services and appointments.
- Conducting assessments and progress monitoring.
- Participating in case reviews and internal staff meetings.
Education:
- Bachelor’s Degree in Social Work or Health Education preferred.
- Combination of experience and education may be considered.
Experience:
- At least 2 years of case management experience.
- 0–2 years of clinical experience.
Licensure/Certifications:
- Valid Driver’s License with good driving record.
- Must be able to drive locally for member visits.
